Mojabi Labs, Inc. d/b/a Wellness Clinic Fresno operates U-IV, a web-based wellness platform that uses artificial intelligence to conduct health intake, generate personalized IV therapy protocol recommendations, facilitate physician review, and manage appointment scheduling. All protocols are reviewed and approved by a licensed physician before administration.
Our service is for general wellness purposes only. We do not diagnose, treat, cure, or prevent any disease or medical condition. IV therapy protocols recommended through our platform and approved by our physician are intended to support general wellness and are not intended to diagnose, treat, cure, or prevent any disease. Nothing in our service replaces the advice, diagnosis, or treatment of your personal physician. You should consult your own healthcare provider before starting any new wellness regimen.
By providing your phone number through our website, patient portal, or booking flow, you expressly consent to receive outbound automated and non-automated SMS messages from Wellness Clinic Fresno (operated by Mojabi Labs, Inc.) related to your IV therapy service. This includes:
We do not operate an inbound SMS intake channel, and clinical intake is not collected by text message. Message frequency varies based on your interaction. Message and data rates may apply. Consent is not a condition of purchase. You may opt out at any time by replying STOP. Reply HELP for customer support where supported.
No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
You must be at least 18 years old to use our service. By using our service, you represent that you are at least 18 years of age. Individuals under 18 require a parent or legal guardian to interact on their behalf.
You agree to provide accurate and complete health information during the intake process, including medications, allergies, medical conditions, and any other information requested. Inaccurate or incomplete information may result in an inappropriate protocol recommendation and could pose health risks. We are not liable for adverse outcomes resulting from inaccurate information you provide.
All AI-generated protocol recommendations are reviewed by a licensed physician before approval. The physician may modify, adjust, or decline any recommendation. The final protocol administered to you is the physician-approved version.
Prices are presented during the onboarding process and may vary based on your personalized protocol. Payment is processed securely through Stripe. Physician review occurs after payment. Refunds are available if: (a) your protocol is not approved by the reviewing physician, (b) you cancel before physician review has begun, (c) treatment cannot be scheduled within 30 days of payment, or (d) you cancel or reschedule more than 12 hours before a scheduled visit. Refund requests should be directed to hello@u-iv.com.
Appointments are scheduled through our online booking system based on nurse availability. We require more than 12 hours notice for full-refund cancellations. Short-notice cancellations (< 12 hours) or no-shows may be subject to a preparation deposit or rebooking fee.
Uploading lab results is voluntary and optional. By uploading, you acknowledge that you have discussed your results with your own physician, you understand their clinical significance, and you consent to our AI and medical team using them to personalize your protocol. You may request deletion of uploaded files at any time.
To the maximum extent permitted by law, Mojabi Labs, Inc. d/b/a Wellness Clinic Fresno shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our service, including but not limited to reliance on AI-generated recommendations or delays in treatment. This limitation does not apply to claims arising from negligence or willful misconduct.
You agree to indemnify and hold harmless Mojabi Labs, Inc. d/b/a Wellness Clinic Fresno, its officers, employees, and agents from any claims, damages, or expenses arising from your use of the service, your breach of these terms, or your provision of inaccurate health information.
These terms are governed by the laws of the State of California. Any disputes shall be resolved in the courts of Fresno County, California.
We may update these terms from time to time. Continued use of our service after changes constitutes acceptance of the updated terms.
